By value, the honest answer is medicine. Pharmaceutical and chemical products are consistently the largest category in Belgian exports, and by a margin that makes the famous consumer goods look like rounding. Part of that is geography doing its usual work. Medicines are high value and low weight, they need reliable cold chain logistics, and they cross borders constantly. A small country with a deep water port, a dense motorway network and an airport with substantial pharmaceutical handling capacity is a natural place to base European distribution, and a large share of what leaves Belgium was not made here. The other part is real research capacity. UCB, headquartered in Brussels, is a mid-sized global biopharmaceutical company built on neurology and immunology. Large international groups run manufacturing and research sites across Flanders and Wallonia. The Flemish institute for biotechnology has spun a stream of companies out of university laboratories, and the clinical trials system is unusually efficient for its size, which is why Belgium has for years hosted more early phase trials per head than most of its neighbours. The concentration comes with the usual warning. An export base dominated by one sector is exposed to that sector's policy risk, and pharmaceutical pricing is a political decision in every market that matters. European reference pricing, American drug pricing reform and the tax treatment of intellectual property all land directly on the Belgian trade balance, and none of them are set in Brussels in the national sense of the word. It also distorts the headline numbers in ways worth remembering when reading them. Because so much of the flow is re-export and intra-company transfer, Belgian trade figures move on decisions taken in corporate treasuries rather than on anything that happened in a Belgian factory.
